That makes this an easy game to explain. If you have played the original, there is nothing new to learn here. What the “1000” buys you is headroom: a single cell on the grid can carry a multiplier eight times larger than before, and a round can pay five times more before it gets cut off. Whether that is worth anything to you depends on how you feel about a game that already asked for patience asking for more of it.

The RTP difference is three hundredths of a percentage point. On £100 of stake that comes to three pence of theoretical return, which is to say nothing at all. How the multiplier spots work
Wins are paid, the winning symbols explode, and the symbols above fall down to fill the gaps. That much is a standard tumble. What Sugar Rush adds is memory.
When a symbol explodes it marks the cell it was standing in. No multiplier appears yet. The second time a symbol explodes on that same marked cell, a x2 multiplier lands there, and every further explosion on top of it doubles the figure: x4, x8, x16, and onward.

Three extra doublings, then. That is where the eightfold jump in the ceiling comes from, and it also explains why the game feels identical for the first half hour. Everything up to x128 behaves exactly as it did in the original.
A multiplier applies to any winning cluster that lands on top of it. Where a single cluster covers more than one multiplied cell, those multipliers add together. They do not multiply each other, and the largest one does not simply win out. A cluster sitting across a x512 and a x256 is a x768 cluster.
The base game comes with a hard limit. Marked cells and their multipliers only survive until the tumbling stops, and once there are no more tumbles the board is wiped clean. The next spin starts from nothing. Stacking a single cell eleven deep inside one tumble chain is close to impossible, which is why the base game rarely produces anything spectacular, and long chains are exactly what a 5 of 5 volatility rating is telling you not to count on.
The free spins round, and why it is the whole game
Free spins are triggered by scatter symbols anywhere on the screen. The same counts retrigger the round from inside it.

The number of spins is not the rule that matters. This is: during free spins, marked cells and their multipliers stay on the board for the entire round and keep growing across every spin in it. The board no longer resets between spins, so a cell that reached x8 on spin two can carry on doubling on spin nine.
That is the only realistic route to the top of this game. Eleven explosions on one cell inside a single base game tumble chain is a rounding error away from impossible. Eleven explosions on one cell spread across a thirty spin round, with special reels in play, is merely very unlikely. Every large Sugar Rush 1000 result you will ever see came out of the free spins.

The bottom right corner of that table is the one to look at. The smallest paying cluster in the game returns 0.2x your stake, so you can hit a win and still lose four fifths of the spin. Cluster games hide their volatility behind a busy board and a lot of small green numbers, and this one is no exception. Those multiplier spots are not a bonus sitting on top of a generous base game. They are the reason the base game is allowed to be as thin as it is.
Is it worth playing instead of the original?
Two identical games with different ceilings is a rare thing to be able to compare cleanly, so the question has an unusually clear answer. It comes down to one preference and nothing else.
Both games take the same money at the same rate, and both pay the same symbols the same amounts. The 0.03 percentage points between their RTPs are not a reason to choose either one. What differs is the shape of where that return lands. Sugar Rush 1000 pushes a slice of its payout into results above 5,000x that the original cannot reach at all, and there is only one place that slice can come from.
